Synopsis

This unit aims to give students broad understanding of the current and future impacts of chronic disease management on primary care, together with knowledge of models and systems of response, and particular skills to use in their workplace.

Outcomes

At the completion of this unit students will be able to:

  1. Describe contemporary understandings of Chronic Disease in Primary Care, especially the demographic changes, lifestyle factors and other social determinants of health.
  2. Describe responses to the challenges of Chronic Disease Management as embodied in models such as the Chronic Care Model (Wagner) and the Australian responses to the challenges of this field.
  3. Demonstrate skills of Motivational Interviewing and in applying the SNAP model in addressing lifestyle issues with patients.
  4. Evaluate their current work-place performance in managing chronic disease utilising audits and a systems approach.
  5. Plan change within their current work-place to improve Chronic Disease Management, utilising their previous evaluations, known effective interventions and published guides, templates and financial modelling.

Students are already working in the field - no extra time is needed for completion of this unit

Workload requirements

12 hours of contact and study time per week.

There are no requirements of on-campus learning.

Private study will involve reading, searching on-line for associated materials, planning and implementing the assessment tasks where indicated.

An MBBS or equivalent, or Master's level in nursing
Must be working in Primary Health Care
